BEHAVIORAL HEALTH CASE MANAGER

Foundations Behavioral Health Services is a community-based behavioral health center serving Mercer County and surrounding areas. Established in 1975, Foundations has been providing professional counseling and support services to local residents for over 40 years. In 2012, the former Gateway Outreach Center merged with Foundations Behavioral Health, allowing us to offer a wide range of mental health and substance use disorder treatment.

 

The behavioral health case manager will provide flexible, mobile, and client-centered behavioral health services to adults and children with serious mental health disorders or substance use disorders in individual, family, or group settings.  The case manager will recognize common symptoms of behavioral health disorders and will develop and coordinate the goals and interventions in the client’s treatment plan.  Will need to work with client and/or family in the natural environment to address specific, individualized goals and interventions and identify and maximize the natural support system and linkage to formal community services.

 

This is a full-time position with work hours generally being day time hours, Monday through Friday.  A flexible schedule is needed in order to meet the needs of the clients and the agency.

Minimum of a high school diploma is required and 3 years of specialized training in case management / social work and behavioral health issues.  Prefer Bachelor’s degree and LSW license through the Ohio Counselor and Social Worker Board and past work experience as a behavioral health case manager.

 

Starting salary is $26,000 – $42,000 annually, depending on education, license and experience.  Full benefits package available, including health and life insurance, voluntary dental and vision plans, retirement plan with agency match, generous vacation and sick leave, 12 paid holidays annually, paid CEU’s, paid license renewal, and a supportive professional work environment.
