Foundations Behavioral Health Services is a cornerstone of community-based behavioral health care, proudly serving Mercer, Van Wert, and Paulding Counties, along with the surrounding areas.

OUR MISSION IS

to provide timely, effective, and efficient assessment, treatment, and support services for the residents of the surrounding areas who are experiencing or are impacted by behavioral health disorders.

Collaboration lies at the heart of our approach. By partnering with various organizations within the communities we serve, we extend our reach to provide on-site services tailored to address a diverse array of behavioral health disorders affecting individuals and families alike. Together, we strive to foster healthier, more resilient communities.

Since our establishment in 1975, we have remained steadfast in our commitment to providing professional counseling and support services to local residents for over 50 years. In 2012, our dedication to expanding access to care led to the merger with the former Gateway Outreach Center, enhancing our capacity to offer a comprehensive spectrum of mental health and substance use disorder treatment options.
